- Complete all information on this form. (download PDF)
- Attach your distributor invoice showing the indicated purchases
of product between February 1, 2007 through May 31, 2007.
You will receive a rebate according to the total number of
cases purchased, with a total maximum rebate of $500.
- Requests must be postmarked on or before June 30, 2007.
- Please allow 6-8 weeks for delivery of payment.
